Exosome Therapy for Acne Scars: What Microneedling Does and What Is Credited to Exosomes

Microneedling has the clearest footing for acne scars: the FDA has cleared at least one device for improving the look of facial acne scars in adults aged 22 and older. That clearance covers the device.

Studies of exosome products for scars are few and mix methods, so what is credited to the exosome is not proven.

What microneedling is cleared for

The FDA's 510(k) letter K241400, dated August 29, 2024, lists two uses for one prescription-use microneedling device: improving the look of wrinkles of the neck for Fitzpatrick skin types II to IV, and improving the look of facial acne scars in adults aged 22 and older of all Fitzpatrick skin types. The device is for trained health care professionals. (FDA 510(k) letter K241400)

This is a statement about a device. It says nothing about any product put on the skin afterward.

What the scar studies compare

A 2026 review pooled 56 randomized trials with 1,488 patients and 34 treatments for moderate to severe atrophic acne scars. It found that combinations generally did better than single treatments, with fractional CO2 laser plus subcision or PRP the most consistent, and it asks for larger, longer trials. (PMID 42429955)

In a 64-person split-face study that was not randomized, the side treated with PRP plus microneedling improved more than the side treated with microneedling alone. (PMID 41460004)

PRP is a different product from exosomes. See the comparison.

What studies say about exosome products and scars

The scar study we cite that tests a topical exosome-type product is a randomized trial of 100 participants with atrophic acne scars. It compared laser plus a topical exosome-mimetic nanovesicle and polynucleotide product with laser alone. At 3 months the scar score fell by 47.3% in the combination group and 35.9% in the laser group. Both groups had the laser, and the product was not an exosome preparation in the strict sense, so the result cannot be credited to exosomes. (PMID 42621380)

None of the studies we cite on acne scars tests an exosome product on its own.

What gets credited to exosomes without proof

  • Improvement that the device alone could produce
  • Improvement that comes from the other product in the bundle
  • Results from a different type of vesicle or from a laser study

When a clinic cites scar results, ask which part of the visit the result came from, and ask for the study.

Before you book

Active acne and skin disease are for a doctor to assess first. Tell the clinic about darker skin, metal allergy, a tendency to form raised scars and pregnancy. The safety guide has the full list. This site covers cosmetic uses only. For any medical condition, talk to a doctor.

Quick answers

Does microneedling help acne scars?

The FDA has cleared a microneedling device for improving the look of facial acne scars in adults aged 22 and older. Clinical results vary, and a 2026 review found combinations generally outperformed single treatments.

Do exosomes help acne scars?

The studies we cite are few and mix methods. In the randomized trial we cite, a laser was used in both groups, so the effect cannot be credited to exosomes.
